Transition Coaching

The transition from a home you’ve lived in for many years to a new community is a huge endeavour.  Navigating new surroundings, making new friends, and dealing with feelings of loss can cause anxiety, depression and a number of other challenges that can negatively impact an individual's health and well-being. 

Aging is Cool Transition Coaching ensures that you, your loved one, or your residents do not have to face their journey alone. Transition Coaches work with residents to identify barriers to adjustment, develop a personalized transition plan, and provide one-on-one supportive visits to help move the resident forward. The program is tailored to the individual resident and emphasizes control, problem solving and accountability.

Why is this important? For those entering a new environment, having preferences heard, and their knowledge and experience respected greatly benefits long-term physical and emotional well-being. 

What We Provide

Skilled Transition Coaches provide 1:1 support to help new residents through a process that is designed to shape greater resiliency, autonomy, internal control and connectedness in their new community. 

The process begins with an initial assessment to determine the challenges and potential barriers to the individual integrating into the community. This assessment is done directly with the resident as well as key family members if the resident desires. Once the plan of action is developed, the Transition Coach will provide individual visits that may include: emotional support, problem solving/coaching, individual activities or accompaniment to group activities. The number of visits and recurrence will be based on the resident's individual needs and availability.

Transition Coaching can be paid for by the resident, family or the community, based on needs.
